GLP-1s and Osteoporosis: Could Weight Loss Medications Actually Protect Your Bones?

When patients ask me about GLP-1 medications such as semaglutide (Ozempic®, Wegovy®) and tirzepatide (Mounjaro®, Zepbound®), the conversation usually begins with weight loss. Increasingly, however, research is suggesting these medications may offer benefits that extend far beyond the number on the scale.
A recently published study in the journal Obesity followed more than 36,000 adults with obesity and found that patients taking GLP-1 medications had significantly lower rates of osteoporosis, major osteoporotic fractures, osteoarthritis, and degenerative disc disease compared with patients taking other weight-loss medications.

The findings were striking. Patients taking GLP-1 medications experienced a 52% lower risk of developing osteoporosis and an 85% lower risk of major osteoporotic fractures compared with patients using other anti-obesity medications.
For years, many physicians have worried that rapid weight loss could weaken bones. This new research raises an intriguing question: could GLP-1 medications actually help protect them?
What the Study Found
Researchers analyzed medical records from over 36,000 adults with obesity who did not have diabetes. Patients receiving GLP-1 medications were compared to similar patients receiving other prescription weight-loss medications.
Compared with non-GLP-1 therapies, patients taking GLP-1 medications demonstrated:
A 52% lower risk of osteoporosis
An 85% lower risk of major osteoporotic fractures
A 66% lower risk of cervical disc degeneration
A 64% lower risk of thoracic and lumbar disc disease
A 55% lower risk of osteoarthritis
Importantly, these findings remained consistent across different age groups, body mass index categories, and in both men and women, suggesting the potential benefits may extend to a wide range of patients.
Why Might GLP-1s Help Bones and Joints?
In my opinion, the answer is likely multifactorial. Excess body weight places tremendous stress on the joints and spine. One of the most common things I hear from patients during their weight-loss journey is that their knees, hips, and back simply feel better. As weight comes off, many patients experience less pain, improved mobility, and a greater ability to stay active.

GLP-1 medications also appear to reduce inflammation throughout the body. Chronic inflammation has been linked to osteoarthritis, degenerative disc disease, cardiovascular disease, and many of the chronic conditions associated with aging.
Researchers are also investigating whether GLP-1 medications may have direct effects on bone metabolism and cartilage health. While the science is still evolving, the possibility that these medications may help support both joint and bone health is generating significant interest within the medical community.
As a physician, I find this particularly intriguing because many patients report improvements in joint pain and mobility long before they have reached their final weight-loss goals. While some of that improvement is undoubtedly due to weight loss itself, researchers are now exploring whether there may be additional benefits occurring beneath the surface.
Why Are Some Studies Showing Bone Loss?
This is where the story becomes more complicated. Not all studies have found protective effects on bone health. Some research has reported decreases in bone mineral density among patients taking GLP-1 medications, particularly when weight loss occurs rapidly.
In my opinion, these findings may not be as contradictory as they first appear. The question may not be whether GLP-1 medications are good or bad for bones. The more important question may be how the weight loss is achieved.
When patients lose weight rapidly without adequate protein intake, resistance training, or proper nutrition, they may lose not only body fat but also muscle mass and bone density.
In other words, the concern may not be the medication itself. The concern may be unhealthy weight loss.
I have long emphasized to my patients that successful weight loss is not simply about eating less. It is about preserving muscle, supporting bone health, and providing the body with the nutrients it needs to remain strong and healthy.
This phenomenon has been observed for years with crash diets, severe calorie restriction, and even bariatric surgery. Whenever significant weight loss occurs, preserving muscle and supporting bone health become critically important.
Muscle Matters
One of the most important goals during any weight-loss program is preserving lean body mass. Many people focus entirely on the number on the scale, but what matters even more is what type of weight is being lost. The goal is to lose excess body fat while preserving muscle.
Muscle is not simply important for strength and appearance. It plays a vital role in metabolism, balance, mobility, insulin sensitivity, and bone health. In fact, maintaining muscle mass becomes increasingly important as we age.
Patients who consume adequate protein and participate in resistance training during weight loss are far more likely to preserve muscle mass and support healthy bones.

This is one reason I believe successful long-term weight management involves much more than simply reducing calories. It requires a comprehensive approach focused on body composition, nutrition, strength, and overall health.
